Introduction:
Adequate management of sedoanalgesia in critically ill patients represents a challenge for nursing due to complications associated with under and over-sedation. Objective: To identify pharmacological principles, evaluate assessment scales, and describe specific nursing interventions in sedoanalgesia management in critically ill patients. Methods: A systematic review was conducted following PRISMA guidelines. Twenty-nine studies selected from 857 records identified in databases (MEDLINE, CINAHL, Scopus, SciELO, LILACS) published between 2018-2025 were analyzed.
Results:
"Analgesia-first" protocols and dynamic sedation were identified as optimal practices. RASS and CPOT scales demonstrated better psychometric properties (κ=0.80-0.91). Nurse-led protocols reduced mechanical ventilation time (2.5 days less, p<0.01), ICU stay (3.2 days less) and delirium incidence (19.4% reduction, p<0.001).
Conclusions:
The nursing role is decisive in sedoanalgesia effectiveness.
