Objective:

To describe a clinical case of bacterial vaginosis in a 36-week pregnant woman in the Ecuadorian highlands.

Case presentation:

A 24-year-old woman, primigravida at 36 weeks according to ultrasound, consulted for moderate hypogastric pain and whitish vaginal discharge that had been present for four weeks. Physical examination revealed greyish discharge without malodour and a closed posterior cervix. Laboratory tests ruled out urinary tract infection. Differential diagnosis established the diagnosis of bacterial vaginosis.

Management and evolution:

Treatment was initiated with 100mg clindamycin plus 200mg miconazole in daily vaginal ovules for seven days, supplemented with probiotics and analgesia. The patient progressed satisfactorily with resolution of symptoms.

Conclusions:

Bacterial vaginosis is a common clinical entity during pregnancy that requires timely diagnosis and appropriate treatment to prevent maternal-foetal complications.
