This study aimed to identify factors associated with acceptance and rejection of kidney transplant offers. A retrospective observational cohort study was conducted with 320 waitlisted patients. Demographic, clinical, transplant offer-related, and psychosocial variables were collected. 46.9% accepted the offer and 53.1% rejected it. Patients who accepted had younger donor age, shorter cold ischemia time, and less ethnic disparity (p<0.05). They also reported lower levels of anxiety and depression, and higher social support (p<0.05). It is concluded that factors related to organ quality and psychosocial aspects significantly influence the decision. Improving patient education, providing psychological support, and optimizing organ allocation criteria are recommended to increase acceptance rates.
