The study aimed to analyse the importance and characteristics of leadership and how it impacts the quality of hospital services. For this purpose, a compilation of national and international research articles from the last four years was conducted using SciELO and Scopus databases, written in English and Spanish. A total of 195 articles related to the subject were identified, 58 of which were related to the topic. In the systematic review process using the PRISMA approach, 22 relevant articles were selected. Relevant characteristics of leadership in healthcare were identified and described, highlighting the existence of complex healthcare problems, the influence of intra- and extra-institutional factors, interpersonal problems, and organizational factors. The aforementioned problems represent challenging challenges for the institutional management of hospitals. It is concluded that leadership has both positive and negative impacts on the quality of healthcare services in hospitals around the world. Therefore, it is recommended that government and management policies include a comprehensive approach to leadership through the promotion, training, and development of future hospital healthcare leaders.
