Advanced medical education faces complex challenges in the context of the 21st century, characterized by accelerated technological advances, the globalization of healthcare, and the growing demand for ethical and humanistic competencies among healthcare professionals. Fellowship programs in oncology represent a key strategy for integrating technical knowledge, clinical skills, ethical values, and complex thinking into the training of specialists, preparing them to face the uncertainty and interconnectedness of contemporary healthcare systems. This conceptual article proposes a prospective vision of advanced medical education, based on a humanistic and ethical approach, where technological innovation and critical reflection are articulated to strengthen professional competence and comprehensive patient care. The methodology adopted consists of a conceptual and prospective approach, supported by a literature review and critical analysis of learning theories, complex thinking, and advanced medical education. Principles of self-regulated learning, complex thinking, and responsible technology management are highlighted as pillars for the training of physicians capable of leading with social sensitivity, ethics, and professionalism. This perspective seeks to guide the evolution of advanced training programs, promoting a balance between technical excellence, ethics, and humanism in clinical and educational practice.
