Health, as a human right, requires trained managers to implement policies and guarantee quality services. This study aimed to analyze the factors that influence managerial competencies in the health field, highlighting their impact on institutional management and performance. A systematic review was carried out under the PRISMA approach, using databases such as Scopus, SciELO and EBSCO, with criteria that included studies published between 2020 and 2024 in Spanish and English. Of 146 articles identified, 26 relevant ones were selected. The results highlight that managerial competencies are influenced by factors such as academic and professional training, soft skills (leadership, conflict resolution, teamwork), the organizational, political and cultural context, and the ability to adapt to digital environments. In addition, coaching and training programs are key to strengthening these skills. It is concluded that the development of managerial competencies is essential to face the challenges of the health sector and improve the quality of services. Elements such as ethical leadership, continuous training, conflict management and international collaboration offer opportunities to optimize the performance of managers and the effectiveness of health institutions.
