Primary health care is essential to ensure equitable and accessible health systems, especially in vulnerable communities. Nursing plays a crucial role in health promotion and disease prevention through educational strategies, comprehensive care, and leadership in implementing innovations. This study aims to analyze the functions of nursing, the barriers faced, and the innovative strategies applied in PHC, using an integrative literature review. Thirty-eight relevant articles were identified in databases such as Google Scholar, SciELO, and PubMed, after filtering 970 initial results. The findings highlight that nursing significantly contributes to reducing risks, improving treatment adherence, and empowering communities through educational and organizational models. However, it faces barriers such as workload, resource limitations, and regulatory restrictions, which affect the quality of care. Innovative strategies, such as virtual care models, educational workshops, and self-managed clinics, have proven effective in overcoming these limitations. This study concludes that, despite the challenges, nursing has a transformative impact on PHC, especially by implementing evidence-based practices that strengthen access and quality of care. It is crucial to continue strengthening leadership, training, and technological integration in nursing practice to ensure more sustainable and equitable health systems.
